Tip

Be certain your child has the required health screenings and immunizations needed to start kindergarten.

The Kentucky Department of Education provides an overview of the required health forms. Also be sure to review information about the Greater Owensboro Summer Feeding Programs.

Required screenings and immunizations:

  • Kentucky Certificate of Immunization Status completed by your child’s physician or a recognized healthcare provider. (diphtheria, whooping cough, tetanus, polio, MMR, Varicella, H influenza type B, Hepatitis B).
  • Kentucky Preventative Health Care Examination Form completed by your child’s physician or a recognized healthcare provider.
  • Kentucky Eye Examination Form completed by an optometrist or ophthalmologist (3 to 6-year-olds).
  • Kentucky Dental Screening/Examination Form completed by your child’s dentist or a recognized healthcare provider (5 & 6-year-olds).

Community Connection

Do you have a pediatrician or family doctor? You will need to schedule a well-visit check-up before school starts. Add a note to your calendar or the refrigerator to remind yourself about making this appointment and to acquire the required forms listed above.
