Be certain your child has the required health screenings and immunizations needed to start kindergarten.
Required screenings and immunizations:
- Kentucky Certificate of Immunization Status completed by your child’s physician or a recognized healthcare provider. (diphtheria, whooping cough, tetanus, polio, MMR, Varicella, H influenza type B, Hepatitis B).
- Kentucky Preventative Health Care Examination Form completed by your child’s physician or a recognized healthcare provider.
- Kentucky Eye Examination Form completed by an optometrist or ophthalmologist (3 to 6-year-olds).
- Kentucky Dental Screening/Examination Form completed by your child’s dentist or a recognized healthcare provider (5 & 6-year-olds).
Do you have a pediatrician or family doctor? You will need to schedule a well-visit check-up before school starts. Add a note to your calendar or the refrigerator to remind yourself about making this appointment and to acquire the required forms listed above.
